PHOENIX — Authorities are investigating a fatal crash involving a Waste Management truck that occurred Monday afternoon in north Phoenix. The collision, which occurred near 40th Street and Shea Boulevard, involved at least three vehicles, including a large garbage truck.

A 44-year-old man, identified as John McLaughlin, was pronounced dead at the scene. Three others, including a child, suffered non-life-threatening injuries and were transported to a local hospital. Fire crews had to extricate one individual from a vehicle at the scene.
